Payment

All services provided are billed directly to the client or caregiver. The client, or caregiver, is responsible for payment of services in full either at the time rendered or monthly, depending upon your payment agreement.  Dr. Hart accepts cash, check, and all major credit cards. Fees and additional financial policies will be discussed when the initial appointment is scheduled.

Dr. Hart will provide detailed monthly invoices with diagnostic and procedure codes needed for your submission to any insurance company.  Payment to Dr. Hart is expected regardless of insurance coverage and/or reimbursement by insurance companies.

Insurance

Dr. Hart does not participate on any insurance panels and is therefore considered an "out-of-network" provider. Many insurers that offer an out-of-network benefit will reimburse for services provided by Dr. Hart. Out-of-network mental health coverage varies significantly per insurance plan. If you plan to pursue reimbursement for services rendered, it is strongly recommended that you contact your insurance company directly, and prior to beginning treatment, to ensure you are fully informed of your reimbursement eligibility. 

Cancellation

Once an appointment is scheduled you are responsible for contacting Dr. Hart should you elect to cancel the session and you are responsible for confirming that your cancellation has been received.

Current Clients: You will be expected to pay the full session fee for appointments cancelled with less than one business days'  notice (24 hours notice during the week; Friday notice for Monday appointments; holidays are treated like a weekend).  There will be no charge for  sessions cancelled with greater than 24 business hours notice. 

New Clients: If for any reason you must cancel or reschedule your initial diagnostic evaluation appointment, Dr. Hart asks that you provide two business days'  notice (48 hours during the weekdays; Thursday notice for Monday appointments; holidays are treated like a weekend). If you cancel with less than  two business days' notice, you will be required to pay half of the cost for the missed appointment.  This fee is nonrefundable. There will be no charge for evaluation appointments cancelled with greater than 48 business hours notice. 

 Be aware that insurance companies do not provide reimbursement for cancelled sessions or any fees related to cancellations.
